So here are 2 samples, taken from a shrubbot file for No Quarter (I suppose this should work for ETPub and maybe Jaymod):

name = (the player name)
guid = (the guid)
ip = (the IP)
reason = (the reason)
made = (the date and time when ban was issued, for instance: 01/13/07 08:24:00)
expires = (0 = perma ban)
banner = (name of the banner)

[mute]
name = (the player name)
guid = (the guid)
ip = (the IP)
reason = (the reason)
made = (the date and time, for instance: 01/13/07 08:24:00)
expires = (0 = perma ban)
muter = (name of the muter)
